Student Award for Interprofessional Collaborative Practice Nomination Form

Nomination submission deadline: August 4, 2025.  

Description: This award is given annually to a student who has demonstrated consistent engagement in an interprofessional collaborative practice within the clinic, classroom and/or community.  

Eligibility: AIHC student member in good standing.

Selection Criteria: 

  1. Should a student wish to be considered for this award they must:
    • Be engaged in collaboration with people from at least one profession other than their own
    • Ask a faculty member or faculty mentor to submit this nomination form and letter of support to include the criteria listed below. 
    • The nominated student must submit an electronic poster to include the criteria below for award consideration
  2. Faculty letters of support should include the following criteria:
    • Description of engagement [project, mentorship, clinical or community experience] in interprofessional collaborative practice.
    • Description of outcomes and/or impact on the clinic, classroom and/or community.
    • Evidence of demonstrating interprofessional communication, collaboration and excellence as a team member.
  3. The nominated student must submit an electronic poster which must include the
    • Description of engagement [project, mentorship, clinical or community experience] in interprofessional collaborative practice.
    • Description of outcomes and/or impact on the clinic, classroom and/or community.
    • Reflection on lessons learned about interprofessional collaborative practice.
    • Description of how experience will impact your future practice.
